GutOptim Side Effects: What to Expect and When to Stop

Written and reviewed by the GutOptim Editorial Desk · Last updated · 9 min read

Quick answerThe most likely GutOptim side effect is mild gas or bloating in the first weeks.

Take each dose with a full glass of liquid, since the fibers swell, and keep medications and iron at another time. It is not for anyone under 18, pregnant or nursing, or with a walnut allergy.

Overview

What side effects can GutOptim cause?

Most adults who react to GutOptim get mild gut effects, not serious ones. Gas, a full feeling or a change in stools are usual with any new fiber, and they often settle.

A few risks are rarer but more serious. They come from specific ingredients: the swelling fibers, the bentonite clay, the aloe leaf, the black walnut hull and the live probiotic. Each one gets its own section below, with what to do about it.

No clinical trial of the finished GutOptim product has been published, so this guidance comes from the label and ingredient research. The ingredients page and the how-to-use guide add more detail.

At a glance

How likely is each GutOptim side effect?

Mild gut effects are the ones to expect. The last rows are rare but need fast action.

“How likely” is a plain-language guide based on the label and ingredient research, not on a trial of the finished product.

Gas and bloating

Why can gas and bloating show up in the first weeks?

Gas is the most expected effect because several GutOptim ingredients are fermented by gut bacteria. Oat bran, apple pectin, flaxseed, glucomannan and plum all reach the colon, where bacteria break them down and release gas.

The American College of Gastroenterology suggests soluble fiber for IBS but warns that fermented fibers can add gas and bloating (ACG IBS guideline, 2021). In a prune trial, 80-120 g of prunes a day raised stool weight and frequency, and also raised flatulence (Lever et al., 2019).

GutOptim holds far less plum than that trial used, since the whole blend is 1.5 g. So heavy gas would be unusual. Our article on why fiber can cause bloating at first explains the process in more depth.

Tip for the first weeks

Take the capsules with a meal and a full glass of water, and spread other fluids across the day. If gas bothers you, try a different meal before you give up.

Choking notice

How serious is the choking risk, and how do you avoid it?

Choking is the most serious risk on the label, and it is easy to avoid. Psyllium and glucomannan soak up water and swell into a thick gel. If they swell before they reach the stomach, they can block the throat.

US rules require over-the-counter products with these fibers to carry a choking warning and to call for a full glass (8 oz.) of fluid (21 CFR 201.319). Health Canada gives the same advice for glucomannan and adds two more points: do not take it right before bed, and do not take it at all if you have trouble swallowing (Health Canada, 2010).

The GutOptim label carries this notice word for word: “This food should be eaten with at least a full glass of liquid. Eating this product without enough liquid may cause choking. Do not eat this product if you have difficulty in swallowing.”

  1. Pour a full glass first

Have at least 8 oz. of water or another drink ready before you open the bottle.

  2. Swallow one capsule at a time

Take each capsule with a good mouthful of liquid. Do not chew or open them.

  3. Finish the glass

Drink the rest of the glass after the second capsule to carry the fiber down.

  4. Stay upright

Take the dose with a meal, sitting or standing. Avoid taking it just before you lie down for the night.

Clay

Can the bentonite clay affect medicines or iron?

Bentonite clay can bind things in the gut, and that may include some medicines and iron. If a drug or supplement is bound, less of it gets into your body. Soluble fibers like psyllium can have a similar, smaller effect.

The simple fix is timing. Take GutOptim at a different time of day from your medicines and iron, and ask a pharmacist how far apart the doses should be for what you take. Bring the label, or the Supplement Facts page, to that talk.

Lead is the other clay concern. In 2016, FDA warned consumers not to use one bentonite clay product after finding elevated lead in it (FDA, Best Bentonite Clay, 2016). That warning covered that one brand only, not all clay. No lead test result for GutOptim has been published, which is listed in the honest note below.

Aloe

What should you know about the aloe vera leaf?

Aloe leaf is the ingredient with the most safety history. The label lists “Aloe Vera (Leaf)” but does not say whether it is decolorized. Decolorized aloe has most of the aloin, the laxative part of the leaf, removed.

Three facts explain the caution. FDA ruled in 2002 that aloe stimulant laxatives are not generally recognized as safe and effective for over-the-counter use (FDA final rule, 2002). A two-year study found large-intestine tumors in rats given nondecolorized whole-leaf aloe extract in drinking water (NTP TR-577, 2013). And the NIH notes that oral aloe leaf extracts have been linked to cases of hepatitis (NCCIH, Aloe Vera).

The amount of aloe in GutOptim is small, since it shares a 1.5 g blend with nine other ingredients. Even so, stop and see a doctor if you notice yellow skin or eyes, dark urine or pain under the right ribs. Our post on bentonite clay and aloe covers both ingredients in detail.

Allergens

Is GutOptim safe with a tree-nut allergy?

GutOptim is not safe for anyone with a tree-nut allergy. The label states: “Contains tree nuts (walnuts).” The walnut comes from black walnut hull, one of the ten blend ingredients.

The facility may also process milk, soy, wheat, egg, peanuts, fish and shellfish, so ask your allergist first if you have a severe allergy. Black walnut hull also has little human research behind it, and safe doses are unknown (WebMD, Black Walnut).

Probiotic

Should people with a weak immune system take the probiotic?

GutOptim includes Lactobacillus acidophilus, a live probiotic. For most healthy adults, common probiotic species cause few problems. People with a weakened immune system are a different case, and doctors often advise extra care with any live bacteria.

That includes people on chemotherapy, transplant drugs or high-dose steroids, and anyone seriously ill. If that is you, do not start GutOptim unless your doctor agrees.

Who should avoid it

Who should not take GutOptim?

Six groups should skip it or get a doctor's view first. The who it is for page covers the fit for everyone else.

Under 18

The label says it is not for use by anyone under the age of 18.

Pregnant or nursing

The label rules out use by pregnant or nursing mothers.

Trouble swallowing

The notice says not to use it at all if swallowing is difficult.

Tree-nut allergy

It contains walnuts from black walnut hull.

Taking medicine

Ask a physician or pharmacist first, as the label advises. Clay and fiber may affect absorption.

A medical condition

Gut, liver or immune conditions call for a doctor's view before you start.

When to stop

When should you stop and get medical care?

Stop and get care

Get emergency help right away (call 911) for chest pain or pressure after a dose, a feeling that something is stuck in the throat, trouble swallowing or breathing, or swelling of the face, lips or throat.

Stop and call a doctor the same day for severe belly pain, vomiting, no bowel movement with a swollen belly, blood in the stool, hives, yellow skin or eyes, or dark urine.

  1. Stop taking it

Set the bottle aside. Do not try a higher or lower dose to test it.

  2. Show the label

Take the bottle, or a photo of its label and lot number, to the doctor.

  3. Tell the order desk

Call +1 (302) 200-3480 with your order ID to report it and ask about the 60-day refund window.

Honest note

What does the label leave out about safety?

A few facts that would help judge safety are missing from the label. The 1.5 g blend does not list how much of each ingredient it holds, so the aloe and clay amounts are unknown. The aloe type (decolorized or not) is not published.

The CFU count and strain code of the probiotic are not published either. The bottle shows lot GUT-26/MY-1730 and a certificate reference, GUT-1730-26-MY, but no lab certificate for lead or microbes has been released. The testing and quality page explains what is and is not known.

FAQ

GutOptim side effects FAQ

What is the most common GutOptim side effect?

Mild gas or bloating in the first weeks. Fermentable fibers and plum can add gas while the gut adjusts. It often eases, but stop if it gets worse.

Can GutOptim make you choke?

It can if you take it without enough liquid. Psyllium and glucomannan swell in water, so take both capsules with at least a full 8 oz. glass. Do not use it if you have trouble swallowing.

Can I take GutOptim with my medicine?

Ask a pharmacist first. The bentonite clay and the fibers may lower how much of some medicines and iron you absorb, so a pharmacist may suggest taking them at different times of day.

Is GutOptim safe with a nut allergy?

No, not with a tree-nut allergy. The label says it contains tree nuts (walnuts), and the facility may also process peanuts and other allergens.

Is the aloe in GutOptim safe?

The label does not say whether the aloe leaf is the decolorized, low-aloin type. Oral aloe leaf extracts have been linked to cases of hepatitis, so stop and see a doctor if you notice yellow skin or dark urine.

Can I take GutOptim while pregnant or breastfeeding?

No. The label says it is not for pregnant or nursing mothers. It is also not for anyone under 18.

Is GutOptim safe with a weak immune system?

Talk to your doctor first. GutOptim contains a live probiotic, Lactobacillus acidophilus, and doctors often take extra care with live bacteria when the immune system is weak.

When should I stop taking GutOptim?

Stop right away and get care for chest pain, trouble swallowing or breathing, severe belly pain, vomiting, blood in the stool, hives or swelling. Stop and call a doctor if mild symptoms get worse or last.
